To ensure a smooth registration process and maximize the time you have to spend with Dr. Singh, please complete the following steps prior to your first visit. We ask that you check-in 10 minutes prior to your appointment in order to complete the registration process.

Step 1 (Obtain Form Password)

In order to access and download our patient forms, please call our offices at 818-889-4532 to obtain our current password.  

Step 2 (Print and Complete All Forms)

Please review, print and complete all of the following forms. You will be expected to bring these completed forms to your first appointment.

Infertility Patients:

• Infertility History Form
• Infertility Patient Registration Form

Gynecology Patients:

• Gynecology History Form
• Gynecology Patient Registration Form

Step 3 (Obtain Additional Information)

Please obtain and bring any medical records which may be pertinent to your visit. This may save you considerable time during the evaluation process. Examples of records that may be useful include but are not limited to:

• Prenatal blood work (blood type, genetic testing, infectious disease testing, etc.)
• Recent pap smear and/or cervical culture results
• Records from any previous infertility evaluation or treatment performed by an outside doctor/office
• Results/films/CD from any pertinent diagnostic procedures (hysterosalpingogram, MRI, ultrasounds)

About Us

Many couples are faced with the deeply personal and potentially devastating challenge of infertility. Often the options that they face in an attempt to have a child can be stressful and overwhelming. Recognizing this, Dr. Anita Singh has established an infertility and IVF facility practice that is staffed with highly trained professionals to provide infertility patients with personalized care second to none.

As the former Chair, Division of Reproductive Endocrinology & Infertility at the Mayo Clinic in Scottsdale, Arizona, Dr. Anita Singh brings her vast experience, outstanding qualifications, and supportive approach to the greater Los Angeles, California area.

Dr. Singh has a unique compassion for her patients. Her personal experience with infertility helps her relate to the struggle that dealing with reproductive issues presents. Listening to patient concerns allows Dr. Singh to create effective individual treatment plans based on her patient's needs. Her goal is to design fertility programs that address each couple's unique medical, emotional and financial need. Dr. Singh's areas of special interests include: In-Vitro Fertilization (IVF), Assisted Reproductive Technologies, Reproductive Surgery (Hysteroscopy and Laparoscopy), Female and Male Infertility, Reproductive Aging, Endometriosis and Recurrent Pregnancy Loss.

LifeStart Fertility Center is located in the city of Agoura Hills in beautiful Southern California. Agoura Hills is in the greater Los Angeles area, only 13 miles from the sunny beaches of Malibu. We are also approximately 60 miles from Santa Barbara. Other neighboring cities less than twenty miles away include Calabassas, Tarzana, Thousand Oaks, Newbury Park, Camarillo, and Simi Valley.

LifeStart Fertility Center of greater Los Angeles has provided IVF and Infertility services to patients locally and from around the world. We Have Moved!

LifeStart Fertility is proud to announce that in early May 2016, our office moved to a new suite within our existing building. Our new office is located in Suite 210 (our old office was Suite 220). We look forward to welcoming patients to this new space!
